Public Health Wales has confirmed that tobacco use costs the country £1.56 billion annually, representing a severe drain on public services, the economy, and local communities. This economic assessment arrives as Welsh authorities prepare to implement the Tobacco and Vapes Act, aiming to eliminate smoking for future generations while targeting youth vaping through a newly launched national consultation.
According to the Public Health Wales report, the financial burden translates to £499 per person, or £4,661 per smoker. Healthcare and social services bear £650 million of this total, which consumes 5% of the national health budget. Lost productivity accounts for another £670 million due to smoking-related illness, absenteeism, and premature deaths. Environmental cleanup, fires, and air pollution add £241 million to the bill.
The report highlights a steep social gradient. Smoking rates in the most deprived Welsh communities stand at 22.6%, more than three times higher than the 6.8% recorded in the least deprived areas. Consequently, hospital admission costs are concentrated in larger, high-deprivation local authorities.

While the National Survey for Wales published in August 2025 shows overall smoking rates have fallen from 13% to 10%, health officials emphasize that more must be done. The upcoming Tobacco and Vapes Act 2026 will introduce a historic ban, making it illegal to sell tobacco to anyone born on or after January 1, 2009, effectively preventing future generations from acquiring the habit.
In addition to tobacco control, Wales is collaborating with other UK nations on a joint consultation to restrict vaping products. The initiative targets bright colors, cartoon branding, and sweet flavor names designed to appeal to minors. Data indicates that 26% of children aged 11 to 16 in Wales have tried vaping, with weekly usage rising to 16% among 16-year-olds.
The public, parents, and health professionals can submit feedback on these proposals through the Welsh Government consultation portal before the October 2 deadline.
