As Dortmund hosts Intertabac, the world’s largest trade fair for tobacco products and smoking accessories, the economic and health consequences of nicotine consumption face renewed scrutiny. While the event draws more than 14,000 international visitors, official public health data indicates that tobacco use imposes an immense financial strain on German society.

A standard pack of cigarettes at a vending machine costs around €10. Although smokers often argue that high excise taxes compensate for public expenditures, federal data reveals a massive deficit between state tax intake and actual economic damage.


Cigarette Consumption and the Rise of Vaping in Germany

Traditional factory-made cigarettes remain the dominant tobacco product in the country. According to the data portal of Federal Drug Commissioner Hendrik Streeck, consumers in Germany smoked approximately 66 billion cigarettes in 2024. Statistically, every German resident consumes nearly 800 cigarettes per year, with gas stations and supermarkets serving as the primary retail channels.

Other traditional products see lower demand. Cigars and cigarillos accounted for 2.3 billion units sold in 2024, while pipe and waterpipe tobacco held minor market shares. In contrast, alternative nicotine delivery systems have expanded steadily. In 2024, retailers sold 1.3 million liters of e-liquids and pre-filled vaping pods.

According to the 2025 Tobacco Atlas published by the German Cancer Research Center (DKFZ), one in four German adults currently smokes. While overall smoking rates among older demographics have declined over the past decade, youth smoking has seen a slight resurgence, with 7% of adolescents aged 12 to 17 reporting active tobacco use.

Young adults represent the highest proportion of smokers. Among individuals aged 18 to 29, over 40% of men and 30% of women use tobacco products. Vaping has grown rapidly in this demographic. Drug Commissioner Hendrik Streeck noted that adult e-cigarette use surged from 2.5% in 2015 to 8.3% in 2024.


The €97 Billion Economic Burden: Healthcare and Lost Productivity

The total annual cost of smoking to German society exceeds €97 billion, according to calculations by the DKFZ. This figure combines direct healthcare expenditures with indirect economic losses resulting from disability and early mortality.

Direct costs include hospital treatments, specialized medications, rehabilitation, and long-term nursing care for chronic conditions. The health insurance provider AOK NordWest reported that hospital admissions for smoking-related illnesses rose by 2.5% in 2024, with over 112,000 patients treated in clinical wards in the North Rhine-Westphalia region alone. Nationwide, 2.3% of all hospital stays in 2024 were attributed directly to smoking-induced diseases.

Indirect costs form the largest share of the €97 billion burden. These stem from lost productivity, short-term absenteeism, and permanent early retirement due to illness. When employees take sick leave for chronic respiratory or cardiovascular conditions, employers bear the direct cost of continued wage payments while non-smoking colleagues absorb the extra workload.

Tobacco Tax Revenue vs. Public Expenditure

The German government collects substantial revenue from tobacco consumption. In 2025, cigarette excise taxes generated more than €13 billion, according to the Federal Statistical Office (Destatis). When combined with taxes on cigars, fine-cut tobacco, and value-added tax (VAT), total annual tobacco tax revenues reach €17.6 billion.

Despite this substantial revenue stream, state receipts cover only a fraction of the economic losses generated by tobacco-related illnesses.

Health Consequences: Chronic Disease and Mortality Data

Smoking remains the leading preventable cause of premature death in Germany. The DKFZ attributes 130,780 deaths in 2023 directly to tobacco use. Furthermore, smoking is identified as the direct cause of 20% of all newly diagnosed cancer cases nationwide.

Chronic Obstructive Pulmonary Disease (COPD) is the most widespread illness linked to smoking. The disease permanently narrows the airways, causing chronic coughing and severe shortness of breath. AOK NordWest registered 817,000 COPD patients in North Rhine-Westphalia alone, attributing 80% to 90% of cases to tobacco consumption.

The health impact of vaping remains an ongoing area of research. While e-cigarettes eliminate the combustion of organic plant matter, the DKFZ warns that inhaling heated chemical e-liquids carries distinct health risks:

“Several substances produced during the heating of e-liquids are carcinogenic or suspected of causing cancer.”

State Protection Measures and Future Tax Escalations

To curb consumption and offset healthcare expenditures, the federal government has scheduled regular increases in tobacco taxation. Under current policy agreements, the average price of a 20-cigarette pack will climb to approximately €11.40 by 2030.

In addition to financial measures, legal protections for non-smokers have expanded. Tobacco smoke contains over 7,000 chemical compounds, at least 250 of which are known toxins or carcinogens. Secondhand smoke causes over 2,000 coronary heart disease deaths and more than 770 fatal strokes among non-smokers in Germany each year.
